The documents are required for each type of waste, for each producer, final destination, and sampling point, and can be issued by the producer of the waste or by the transportation company.

These documents are an integral part of the records for loading and unloading of the waste produced and managed.
Article 93, paragraph 2 of Legislative Decree 152/2006 requires the copies of the form be kept for 5 years.

- Characteristics of the waste (codified description indicated in the European waste catalogue)

- Physical state (tick box)
- Hazard characteristics (tick box)
- Number of packages (indicates the number of transported units)li>
- Destination (reports whether the waste is destined for recovery or for disposal, with the relevant code)
- Chemical-physical characteristics (only in case of disposal in landfills)
- Quantity, weight and destination (the quantity of transported waste indicated by the manufacturer can be due to the nature of the waste or to the unavailability of a weighing system, and thus can be an approximation)
- Transport subject to ADR / RID regulations (ADRF for road transport, RID for rail transport)
- Signature of the producer and transporter (the signatures must be indicated in the box)
- Surname and name of the driver (employee of the transportation company)
- Vehicle license plate (referred to the vehicle used for transportation)
- Trailer license plate (if applicable)
- Date and time of transport (must report the time of departure, the date of departure cannot match the emission date)
- Reserved for the recipient (the recipient must indicate if the waste was accepted or rejected)
- Date (date of arrival)
- Signature of the recipient (the recipient must sign and stamp for the responsibility of the information reported in the form)
